In the climax of The Disappearance, Kyon is stabbed by Disappearance Ryoko Asakura, and when he closes his eyes, he wakes up in a hospital, back in the regular world. Next to him is Koizumi, as it was his turn when this happened (most SOS Brigade members took turns, except for Haruhi, who didn't move at all). He was peeling apples, probably to kill time. This fanart exaggerates the amount of apples he peeled, and planned to peel for comedic purposes.
